Postmenopausal women with prediabetes. 250mg NMN daily for 10 weeks. Significant improvement in insulin sensitivity in skeletal muscle specifically. NAD+ in muscle tissue increased ~90%. No significant change in body composition, fasting glucose, or other metabolic parameters. Robust small RCT — well-controlled. Key limitation: prediabetic population, short duration, surrogate endpoints only.

Yi et al. — Frontiers in Aging, 2023

Healthy middle-aged adults 40–65 years. 300mg or 600mg NMN daily versus placebo for 60 days. Dose-dependent increase in NAD+ levels confirmed. Improvements in muscle strength at 600mg. No significant change in inflammatory markers, blood lipids, or cognitive function. Tolerated well at both doses.

Igarashi et al. — NPJ Aging, 2022
n=30
Amateur runners 40–65 years. 250mg NMN daily for 6 weeks. Improved oxygen utilization and ventilatory threshold during exercise testing versus placebo. Mechanistically consistent with NAD+-dependent mitochondrial function enhancement in exercising muscle. Not a longevity population. Hard endpoints absent.
§ The honest clinical position

Evidence tier: Emerging. The case for clinical consideration rests on the mechanistic coherence and the published surrogate-endpoint human trial signal. The case for caution rests on the absence of hard longevity outcome data in adequately powered trials.
