Jardiance (empagliflozin) and longevity: reading the brand-name evidence

Jardiance is a brand name for empagliflozin, one of the two SGLT2 inhibitors with the strongest cardiovascular outcome data. The evidence belongs to the molecule and the drug class, not the brand — but the distinction matters for how you evaluate what you read.

§ Brand name, generic molecule, drug class

Jardiance is the U.S. and EU brand name for empagliflozin, a sodium-glucose cotransporter-2 (SGLT2) inhibitor. All of the pivotal trial evidence for this molecule — EMPA-REG OUTCOME, EMPEROR-Reduced, EMPEROR-Preserved — was conducted using empagliflozin under its generic name, and the FDA-approved indications (type 2 diabetes, heart failure, chronic kidney disease) attach to the molecule, not the brand specifically.

This distinction matters because searches for "Jardiance and longevity" or "Jardiance anti-aging" are really asking about empagliflozin's — and by extension the SGLT2 inhibitor class's — broader healthspan evidence, which Geroevidence tracks at the class and molecule level.

§ The landmark trial

EMPA-REG OUTCOME (2015) remains the foundational trial: empagliflozin reduced cardiovascular death by a hazard ratio of 0.62 versus placebo in adults with type 2 diabetes and established cardiovascular disease — one of the strongest hard-outcome results tracked for any intervention on Geroevidence's index. This is the trial result underlying SGLT2 inhibitors' Strong evidence tier.

Subsequent trials (EMPEROR-Reduced, EMPEROR-Preserved) extended the evidence to heart failure across the full range of ejection fraction, further broadening empagliflozin's outcome-level evidence base beyond its original diabetes indication.

§ What remains untested

All of empagliflozin's pivotal outcome data comes from populations with diabetes, heart failure, or chronic kidney disease — not from healthy, non-diabetic adults using it specifically for longevity or healthspan purposes. Evidence in that specific population is considerably more limited, a caveat that applies to the SGLT2 inhibitor class broadly and not to empagliflozin uniquely.

§ The clinical takeaway

Jardiance's underlying molecule, empagliflozin, has some of the strongest hard-outcome cardiovascular evidence of any drug tracked on Geroevidence — but that evidence comes from diabetic, heart-failure, and CKD populations, not from healthy adults using it off-label for longevity. Evaluate the molecule and its trial populations, not the brand name.
